Syndrome of Inappropriate ADH Secretion (SIADH). Associated with pulmonary tumors which can secrete ADH, CNS disease, and certain drugs. Excess ADH stimulates the renal tubules to reabsorb water and release highly concentrated urine. This results in hyposmolarity, hyponatremia, cellular swelling, edema, neuro dysfunction. Water restriction and diuretics can be used to manage hyponatremia. Management of the underlying cause is of utmost importance.

I'd try to stay away from distal acting diuretics given hyponatremia. But, I don't disagree with this approach. Decongest by any means necessary! HypoNa less important

Electrolyte like Na (hypernatremia) actually contribute to HBP and most times needs to be reduced via diuretics. Incase of fluid overload in hyponatremia, Aquaretic will suffice

3. Replete Mg2+ and K+ (aggressively). Don't forget that even normokalemia 3.5-4.0 mmol/L often indicates massive total body K+ depletion in patients on diuretics. Depletion moves Na+ inside cells to maintain osmotic equilibrium, worsening hyponatremia. Repleting:⬆️Na+

Sometimes hyponatremia can indicate total body sodium deficiency & sometimes not. Most common cause of hyponatremia in elderly is diuretics.

Seen 73 yr old patient in cardiac services unit. Depressed. What checklist considerations come to mind when thinking of ADs and cardiac unit patients. I'll start - SSRIs and SIADH ( hyponatremia) exacerbated by prescription of diuretics.

Depends...the greatest cause of hyponatremia in the elderly is sodium loss via diuretics.
