Syndrome of Inappropriate ADH Secretion (SIADH). Associated with pulmonary tumors which can secrete ADH, CNS disease, and certain drugs. Excess ADH stimulates the renal tubules to reabsorb water and release highly concentrated urine. This results in hyposmolarity, hyponatremia, cellular swelling, edema, neuro dysfunction. Water restriction and diuretics can be used to manage hyponatremia. Management of the underlying cause is of utmost importance.

Best Edema Diuretics: Loop vs. Thiazide
Diuretics help the body get rid of water through diuresis or increased urination. Some people refer to these medications as ‘water pills.’ There are several types of diuretics. Here we compare two of these medications, loop and thiazide diuretics. #waterpills #edema #waterweight #diuretics #swelling

Low Blood Sodium (Hyponatremia)
Low blood sodium, or hyponatremia, occurs when water and sodium are out of balance in your body. It can cause weakness, headache, and muscle cramps. Cutting back on fluid intake and adjusting the dosage of diuretics you take are just two ways you can treat low blood sodium. Prevent hyponatremia by keeping hydrated.

the one that gets me is thiazide diuretics. Most deep dives into the mechanism show weight gain rather than weight loss with thiazide induced hyponatremia. see:

1/ Omeprazole(and other PPIs) can cause hypomagnesemia and hyponatremia. 2/ Diuretics can cause hyponatremia, hypokalemia(especially loopdiuretics)

Why thiazides causing more hyponatremia than loop diuretics? Interesting study

Common side effects with thiazide diuretics: • Hyponatremia • Hypokalemia • Hypercalcemia – thiazides increase calcium reabsorption from distal tubule.

Diuretics - the number of elderly on diuretics with significant hyponatremia or recurrent hyponatremia in the people I see has always been striking.

I'd try to stay away from distal acting diuretics given hyponatremia. But, I don't disagree with this approach. Decongest by any means necessary! HypoNa less important

💊✅Side effects of thiazide diuretics include hyponatremia, hypokalemia - which may predispose to cardiac arrhythmias, increase risk of gout, and they may cause impotence in men.

Typical question on rounds: “Why do THIAZIDES cause MORE HYPONATREMIA than loop diuretics?” 👉🏼

Prinztruestar Apr 12
Electrolyte like Na (hypernatremia) actually contribute to HBP and most times needs to be reduced via diuretics. Incase of fluid overload in hyponatremia, Aquaretic will suffice

Great article. I'm going to bring it to a meeting this am. Seeing a lot of hyponatremia secondary to diuretics

Strong ion difference seems normal (Na-Cl). Any other causes for hyponatremia (diuretics, transient SIADH, no more intake of solutes - just drinking)?

3. Replete Mg2+ and K+ (aggressively). Don't forget that even normokalemia 3.5-4.0 mmol/L often indicates massive total body K+ depletion in patients on diuretics. Depletion moves Na+ inside cells to maintain osmotic equilibrium, worsening hyponatremia. Repleting:⬆️Na+

yeah just spent 1 hour opening pharmaco book and revising why thiazides cause higher hyponatremia than loop diuretics :/

drjamesdinic 13 Jun 2017
Sometimes hyponatremia can indicate total body sodium deficiency & sometimes not. Most common cause of hyponatremia in elderly is diuretics.

RosenelliEM Apr 24
-cirrhosis is a sarcopenic/poor nutrition/catabolic state in many cases -anyone with persistent hyponatremia either from diuretics or other causes are at risk for developing osteoporosis as the your bones will start to buffer the Na

DrToddLee 10 May 2018
But. But. My eight glasses a day!! Wonder though -- was there a difference in rates of hyponatremia or need to diuretics? Was quality of life better?

Seen 73 yr old patient in cardiac services unit. Depressed. What checklist considerations come to mind when thinking of ADs and cardiac unit patients. I'll start - SSRIs and SIADH ( hyponatremia) exacerbated by prescription of diuretics.

drjamesdinic 10 Feb 2018
Depends...the greatest cause of hyponatremia in the elderly is sodium loss via diuretics.
