Syndrome of Inappropriate ADH Secretion (SIADH). Associated with pulmonary tumors which can secrete ADH, CNS disease, and certain drugs. Excess ADH stimulates the renal tubules to reabsorb water and release highly concentrated urine. This results in hyposmolarity, hyponatremia, cellular swelling, edema, neuro dysfunction. Water restriction and diuretics can be used to manage hyponatremia. Management of the underlying cause is of utmost importance.

Hyponatremia with elevated tonicity can occur with high blood sugar. In hypertonic hyponatremia, resorption of water by molecules such as glucose (hyperglycemia or diabetes) or mannitol (hypertonic infusion) occurs.
