The Sodium Correction for Hyperglycemia Calculates the actual sodium level in patients with hyperglycemia. Hyperglycemia causes osmotic shifts of water from the intracellular to the extracellular space, causing a relative dilutional hyponatremia. The classic correction factor of 1.6 mEq/L for every 100 mg/dL increase in serum glucose was challenged by a paper by Hillier et al in 1999, and sometimes their factor of 2.4 mEq/L is used.

On hyponatremia and postop outcomes: "Hospitalists [should not] recommend that all patients with hyponatremia delay surgery until their sodium imbalance is corrected. Instead, finding hyponatremia can help doctors identify a subset of patients who may need increased perioperative surveillance." Alexander A. Leung, MD, University of Calgary

Central Pontine Myelinolysis (aka OSMOTIC DEMYELINATION)... This occurs if you correct HYPOnatremia too rapidly... Correcting HYPERnatremia too quickly results in Cerebral Edema... Na+ should only be either lowered or raised 0.5mEq an hour and no more than 12mEq a day
CORRECT ANSWER: C) Diabetes insipidus: hypernatremia often develops due to the dehydration that this diagnosis causes A) Addison's disease: one of the symptoms of this disease is actually craving salty foods. These individuals have abnormalities with the production of hormones by the adrenal glands. Specifically a mineralocorticoid such as aldosterone. This hormone helps to regulate sodium balance in the body. B) Diarrhea D) Hyperglycemia E) Alcoholism #sodium #hyponatremia #lowsodiumcaus...

4/15 This is a tricky question ⏳In a 24 hour period; 🔹The goal for correction is 4-6 mEq/L (expert opinion) 🔸While the limit for correction is ≤ 8 mEq/L

Again a warm thanks from me Doc T! Just so happens we had this lesson in our surgery subject and recently learned that rapid hyponatremia correction leads to pontine demyelination! I never expected such info would be anything more than trivia!

Hi so good to see u here in Asia. I want to ask you why the rules of hyponatremia correction dont hold true for hypernatremia? whats the physiological basis?

I’m so sorry my sodium censors were off and I missed the call! I think a lot of providers would have made same/similar decision, they’re tough!! Hyponatremia correction seriously makes my brain hurt.

scoca1 15 May 2019
Also, sure shoot for a target (e.g., 12 mmol/L/day). But, DON'T SWEAT it like we do in hyponatremia correction if it is not meeting your exact target. It won't matter if it's 15 one day and 10 the next.

AliThaver 29 May 2019
When I explain hyponatremia correction to my patients I often start by saying: ‘it’s about the journey and not just the destination’. My patients are instantly on board!

I could get into the medicine more, as has kindly done re: hyponatremia correction, but what would be the point? It would just make me more irate. This is NOT the hill you want to die on, these are not the martyrs you want representing your crazy-ass cause.
